General issue: footnotes have lost their indicators (in tables and otherwise) #9

Footnotes have problems. See Table 2.12 as an example. The footnote indicator ("a" in this case, was "1" in the original document) does not appear with the footnote text. Same problem in a non-table context on page 8, where the "1" does not appear with the footnote text.

Hm, that link is broken for me. In any case, the footnotes have mostly been fixed one way or another, so I will close this. There are some individual cases I will still take a look at fixing.
