Table captions lost when tables continued across pages #8

With a table that does break across a page (like 2.11), we've lost the ability to repeat the table caption (original: "Table 2-11. Scalar Types (Page 2 of 2)"). Maybe that's ok, but probably it's better to have at least some indication that this is a continuation.

This is not supported in Apache FOP yet:
